Job Details:
The Skilled Care Social Worker is responsible for managing a high volume caseload working with patients and their families with complex issues. As a patient advocate, this person works with the patient to assist them with social, emotional, and economic needs in collaboration with the interdisciplinary team both internal and external to the facility.
Behaves in a professional, customer-focused, service-oriented manner; displaying and promoting respect, care and dignity for all internal and external customers; facilitates a team-oriented attitude, striving continuously for service excellence.
Qualifications/Competencies
Minimum of LSW, Conditional
LSW preferred
Bachelor's degree, BSW or equivalent in related field is strongly preferred
1 year of experience preferred
