Job Description:
We are looking for a Registered Nurse (RN) Hospice Team Leader that can provide leadership, guidance and support to the clinical staff as well as manage a caseload of hospice patients and provide nursing care.
You will be responsible for educating patients and their families through this difficult time in their lives as well as mentoring colleagues in your discipline.
The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision.
Responsibilities:
Provide visiting hospice care services to an assigned caseload including admissions, re-certification and discharges.
This position assists leadership with ongoing education and training of all clinicians to ensure understanding of documentation requirements to meet regulatory standards.
Develop the Plan of Care in coordination with all health care professionals assigned to the case.
Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges.
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
Manage and supervise the interdisciplinary team.
Occasional weekend coverage & holidays (as needed).
Qualifications:
Licensed Registered Nurse in the state of Maine with at least 1 year of clinical experience; Hospice preferred.
COVID Vaccinated Oasis experience strongly desired Have a valid driver's license, a reliable vehicle and willing to commute to provide the best in home care.
Willing to work one weekend (Saturday and Sunday) per month with one (1) day off leading into the weekend and one (1) day off leading out of the weekend.
Willing to work one (1) winter holiday and one (1) summer holiday per year.
