Job Details:

Working closely with Patient Services, coordinates communication among all disciplines involved in the care of the patient and care management activities and facilitates development of the interdisciplinary plan of care. Also serves as a key contact for a patient's care coordination, utilization management and discharge planning coordination.
May perform all three core care management functions (care coordination, UM and discharge planning) in an integrated manner or, in a float capacity may perform one or more of these functions and/or other psychosocial support as a resource care manager in support of the overall care management.
Qualifications/Competencies:

Licensure requirements:

MaineStateBoard of Social Work Licensed Master Social Worker (LMSW)OR

MaineStateBoard of Social Work Licensed Clinical Social Worker (LCSW)OR

Maine State Board of Social Work Licensed Social Worker (LSW) with at least 3 years of experience specific to behavioral health settingOR
Must have either a current State of Maine Registered Nurse (RN) license or an RN license recognized by the Nurse Licensure Compact ( ). If RN licensure is from a compact state, must obtain a state of Maine RN license within 90 days of changing primary residency toMaine.
ACM, CCM, ANCC or other nationally recognized case management certification

Bachelor's degree in Nursing orBachelor's degree in Social Workpreferred

3 years or more of experience in clinical or case management required.

Must have strong familiarity or direct experience with the following: case management, care coordination, discharge planning, utilization review, health care setting(s), medical terminology, disease processes, collaborative practice,

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
