Job Details:
The Case Manager is responsible for managing complex patient issues in relationship to needed services and the patient's transition through the healthcare system. As a patient advocate, this person works collaboratively with patients and families and with the inter-disciplinary team both internal and external to the organization
Qualifications/Competencies:
Maine State Board of Social Work Licensed Social Worker (LSW) or Licensure eligiblerequiredor
Must have either a current State of Maine Registered Nurse (RN) license or an RN license recognized by the Nurse Licensure Compact ( ). If RN licensure is from a compact state, must obtain a state of Maine RN license within 90 days of changing primary residency to Maine.
Bachelor's degree, in Social Work or related field preferred
1 year or more preferred
Utilization Review experience preferred
Proficiency in a suite of MS Office applications preferred
